I.  CONTRACT/ AGREEMENT

1.1   Provides information on:

       A.  Name & Permanent address of the guest/ resident

       B.  Terms and Conditions

b.1  Monthly rental based on bed occupied, shall  include basic electricity and water  consumption.

b.2  All payments must be made within the first five days of the month. 

b.3  A penalty of 15% monthly shall apply to late payments and/or delinquent accounts.

b.4  2 months Security Deposit (based on monthly  rental) is due and demandable upon signing of contract.  Refundable, between 10 to 15 working days, after end of contract but subject to conditions and/ or deductions whenever  applicable.  It shall not be applied on any unpaid rentals  and/or utility charges.

b.5  2 months Advance Payment (based on monthly rental) is also required upon signing of contract.  Applicable on the first and last months of the contract.

b.6  The period covered by each agreement shall start from 12 noon of the first day of contract and 12 noon after the last day stipulated on the contract. Accommodation requirement before and/or after the contract period shall be considered as transient (or short term), and therefore prevailing daily or short term rates shall apply.

b.7  Breach and/ or default by the resident with regards to any stipulation within the contract shall mean automatic cancellation of the agreement and forfeiture of all deposits and payments.

b.8 Any amount due to the dormitory shall immediately become due and demandable

b.9 The resident shall be governed by the establishment's policies, rules and regulations, both existing and new.  It shall be the resident's duty to be acquainted with the aforementioned and to abide by them.

b.10  Once application is approved, the resident shall accomplish the standard registration form and submit all requirements as required by Management.

b.11  The management reserves the right to cancel this agreement provided a 30-day written notice is given to the resident.

C.  For residents under 21 years old, the signature of guardian is required upon signing of the agreement.

D.  Both the resident and The General Manager shall sign the agreement.  The resident shall keep her own copy of the agreement.

  

II. MOVING-IN

No resident and/ or transient shall be allowed to move in without a duly accomplished move-in slip authorized by Management.  The penalty for violating this policy is outright dismissal.

III. MOVING- OUT

All concerned dormitory residents must advise the Dormitory  Office of plans to move out at least (15) fifteen working days before the intended date.  Furthermore, it is expected from the individual concerned that all of his/ her accounts are settled and appropriate clearances and forms are accomplished within the specified period.

IV.  TERMINATION OF AGREEMENT

Pre-termination of lease by resident is permitted.  However, this is subject to the following conditions:

a.  With written notice at least thirty (30) days prior to conclusion of the lease; fifty percent (50%)    forfeiture of any and all deposits and/ or payments.

b.  With written notice but less than thirty (30) days prior to conclusion of the lease; seventy five percent (75%) forfeiture of any and all deposits and/ or payments.

c. Absence of notice, written or oral, shall be considered breach/ default; one hundred percent (100%) forfeiture of any and all deposits and/ or payments.

d.  Any amount due to the dormitory as incured by the resident shall be immediately due and demandable.

          The management reserves the right to cancel/ terminate lease agreements provided a thirty (30) day written notice is given to affected parties.  If any, advance payments and/ or deposits less any incurred charges and/ or balance owed to the dormitory shall be refunded to the individual concerned.

V.    MODE OF PAYMENTS

a.  Monthly rentals shall be deemed due without the need for demand on the first day of every month.

b.  For residents availing Monthly Payment Scheme, a statement of account shall be given seven (7) days prior to the start of each month.

c. Five (5) days shall be established to which a resident may his/ her account, failure to do so shall mean a penalty fee of fifteen percent (15%)  of the amount due applied to current billing.

d.  Any unpaid rental over thirty (30) days shall be considered a breach of the agreement and therefore subject to immediate termination of lease.

VI. OFFICE HOURS AND PAYMENT TRANSACTIONS

The Dormitory Office shall be open for business daily from 8am up to 5 pm.  It shall be closed during National and Local holidays.
Residents and guests must transact their business within the abovementioned period.
Residents and guests must always demand for a receipt whenever settling accounts.


VII.SECURITY SERVICES

The dormitory is provided with security services.  To help the security personnel to protect us, please take note of the following:

1.  The guard on duty will also request all visitors to identify themselves, to mention the party they wish to see, and give them other necessary information.

2.  The security guard are not responsible for loss of valuable and other personal belongings of residents like jewelry and cash or check.  To prevent or minimize such losses, we request that:

a. Each resident must exercise care in keeping her personal belongings and not carelessly leave them in public places such as at the telephone booth or in the lobby and lounge washroom or on tables in the hallways.

b. Personal electronic equipment such as radio, TV sets, and other allowed appliances brought into the dormitory must be registered in the office and proper gate passes must be secured from the office staff/ guard on duty when these items are brought out.  UNREGISTERED ITEMS ARE NOT ALLOWED TO LEAVE THE BUILDING.

3.  Security of the resident's things inside the room is the responsibility of every individual.

4. The right to inspect residents and guests’ leased premises shall be reserved and exercised by the management and/or its representatives whenever it is deemed necessary.

VIII. RECEIVING OF VISITORS  AND  CURFEWS  OF RESIDENTS

Residents may entertain only at their respective lounges between 9am and 9pm, Mondays thru Sundays. Should the need arises and depending on circumstances, a resident may be allowed to receive a visitor outside the prescribed period provided a permit is obtained from the Dormitory Supervisor.

The curfew for all residents shall be 12 midnight any day of the week.  This policy may be waived in instances wherein a resident is required to stay out later that the prescribed hour.  However, a letter from the parents giving their child permission to stay out late must be presented.

The penalty for residents violating the policy on curfews shall be:

1st Offense:  Written  reprimand  and  parents  of resident concerned shall be notified.

2nd Offense:  Lock  out  from  the   premises  and parents of resident shall be notified.

3rd Offense:  Termination of lease agreement.

Working Professionals may come home on their own schedule especially when the nature and schedule of their work necessitates it. But they should inform the dormitory office about their work schedules.

All residents must leave their respective room keys with the Dorm Office whenever he or she will be out of the premises for more than twenty-four hours.

Duplication of room keys is strictly prohibited.  Residents found in possession of unauthorized key copies will be penalized   Php 500.00 for each copy made.

The Dormitory Office must be notified immediately whenever a resident loses a room key.  The fee for lost key is Php 1,500.00 per key.

IX. LODGING ASSIGNMENTS

Accommodation assignments, both at the beginning of the lease agreement and during the resident's stay shall be solely Management's prerogative.

Nevertheless, the individual needs and requests of the resident may be considered in rooming assignments and/or dorm mate selection/pairing.

Residents are responsible for the proper maintenance of their leased premises, ensuring that it's both sanitary and free from noxious odors.  Residents are also obliged to observe proper care for the whole of the facility.


X.  APPLIANCES & ELECTRONIC EQUIPMENTS

Residents may bring in personal appliances for their convenience. Naturally, these must be declared with the Dormitory Office and shall be subject to prevailing charges. 

However, Management reserves the right to disallow and/or disapprove any and/or all of said equipment, device and/or appliances of the resident should this prove disadvantageous to other parties and/or if appropriate charges are not settled.

REMINDER:
Never plug in an electric  appliances with a 110 voltage unless you have a corresponding transformer. 

XI.  LOUNGE

A tv set is provided for both floors for the recreation of all residents.  This may be tuned in by residents who would like to view some programs.  Residents may stay at the lounge until 12 midnight only.

XII. MEALS AND SNACKS

Cooking inside the room/unit is strictly prohibited as this constitute a fire hazard. 

Residents may have their meals and snacks inside their own rooms, that they maintain the cleanliness of their rooms.

XIII. SMOKING AND DRINKING OF ALCOHOLIC BEVERAGES

Smoking, pursuant to Republic Act 9211, is strictly prohibited in any part of the dormitory premises.

Consumption and/ or custody of any type of alcoholic beverage are strictly prohibited in any part of the dormitory premises.

The penalty for dormitory personnel violating said policies shall be:

1st Offense:  Smoking–Seven (7) days suspension w/o pay.
                      Drinking -  Termination

2nd Offense:  Smoking -  Termination

XIV. LAUNDRY

Residents are not allowed to do their laundry in the dormitory premises.

XV.  TELEPHONES

Telephones at the guard shall not be used for outgoing calls of residents.  Pay telephones are installed at both floors for out going calls of residents.

XVIII. SECURITY & SAFETY MEASURES

We have taken every precaution to ensure your comfort and safety.  However, in the unlikely event of an emergency due to fire, it is best for everyone in the building to be familiar with basic life safety recommendation.

  * FIRE SAFETY MEASURES

      In case of any visibility problems due to heavy smoke   during fire, please observe the following pointers:

  • Count the number of doorways between your door and emergency exit.
  • Check the location of fire extinguishers/hoses in the corridor of your floor.
  • Study the air-conditioning controls in your unit (On-Off switch)
  • Remember smoke always rises.  Keep crouched to the floor if corridors are smoke filled.

  * PREVENTING A FIRE

  • Stop the use of appliances in your room.
  • Never smoke in bed, inside your unit and within the dormitory premises.
  • Un plug all electrical appliances before retiring for the night.

  * IF YOU DISCOVER A FIRE…

  • Extinguish small flames with the nearest fire extinguisher and report your action to the guard on duty and dormitory office.
  • Leave the building, if possible.  Take all your keys with you.
  • If you are unable to leave your room, close the door(s) to keep out the fire.
  • Place a towel along the button of entrance door.

* IF A FIRE STARTS IN YOUR ROOM/ UNIT…

  • Stay calm, turn-off the air conditioner.
  • Evacuate your room immediately. 
  • Do not stop to take any possessions with you.
  • Close the door securely to inhibit the spread of  fire.
  • Activate the nearest fire alarm and notify the guard on duty.  Walk to safety via the emergency exit.
  • Do not use the elevator.

For your information, the dormitory's Fire Security Systems consist of:

  • Sprinklers
  • Fire Extinguishers
  • Alarm Bells
  • Break Glass Units
  • Smoke and Heat Detectors

* EVACUATING IN CASE OF FIRE:

  • Take all your keys and move cautiously.
  • Feel the door at the back of your hand.  If the door is hot or abnormally warm, Do not open it.  If the door is not hot, open it carefully.  Close the door behind you to prevent spread of fire.
  • If there is smoke in the corridor, stay as low as possible.  Crawl on your hands and knees if necessary.
  • Proceed to the nearest emergency exit.
  • Do not use the elevator.
  • If your exit is blocked, go back to your room or proceed to the roof. Stay calm and always hold onto the handrail.  Leave the exit door to the roof open.  This is the only door that should be left open in an emergency.
